Service Description

You will be asked to provide a Nutrition Intake Form as well as a Food Frequency Questionnaire prior to your consultation appointment. We will review your current diet and goals, and address any issues or barriers you may have. I will provide nutritional recommendations based on your lifestyle, goals, and preferences.
